We take referrals from General Practice and other Neurology consultants
We will see you in the Outpatients Department in the West Wing, John Radcliffe Hospital or at Bicester Community Hospital.
To assess the referral for an appropriate clinic we will need the following
A list of all the treatments you are currently taking:
What medications are you currently taking for your headaches? (MS Word, 14 KB)
A list of all the treatment you have tried in the past for your headaches:
What preventive medications have you tried in the past for your headaches? (MS Word, 14 KB)
A diary of your headaches (minimum over the previous three months):
