We aim to provide the same level of rehabilitation in the community as that provided on an inpatient Stroke Unit.
The team comprises:
- Stroke Consultant
- Physiotherapists
- Occupational Therapists
- Speech and Language Therapist
- Stroke Nurses
- Clinical Psychologist
- Rehabilitation Assistants
Each patient is set specific goals, which take into account the needs and abilities of their carer or carers.

Patient eligibility criteria
Patients must:
- have a diagnosis of a new stroke. If the patient has already had a period of inpatient rehabilitation of up to one week, please contact the team to discuss your referral. We do not routinely accept referrals from inpatient rehabilitation settings
- be registered with an Oxfordshire GP or pay their Council Tax to Oxfordshire County Council: any medical needs should fall within the remit of the GP
- consent to ESD and agree to participation in a goal-focused rehabilitation programme
- be assessed as safe to return home by the referring multidisciplinary team
- be continent, or have a continence plan in place
- have care needs that can be supported by family or a community support agency e.g. Home First Oxfordshire. Appropriate care should be ready to start upon discharge
- have a safe transfer method established and appropriate equipment in place prior to discharge.
