- where there is pharmacokinetic variation such that concentrations in blood cannot be predicted
- to confirm oral absorption

Vancomycin, gentamicin and amikacin TDM assays are performed in the OUH Biochemistry Laboratory.
Other antibiotic / antifungal TDM are sent to reference laboratories by Microbiology.
