Ideally patients admitted to hospital will be seen before discharge by the Heart Failure Nurse Specialists. Written and verbal information is given regarding signs and symptoms, lifestyle advice and when to seek help.
If the patient is not seen before discharge, and they meet referral criteria, relevant information is sent to the Community Heart Failure Nurses in Oxfordshire and surrounding counties:
